hud options--> config properties--> turn on show note icon and turn on 'show auto rate' .
You'll now have two icons in the HUD, the 'N' icon and a '*'. Click on the * to add notes.

That is how my config was setup. When I click the autorate icon (or * if not assigned yet) it brings up a drop-down menu to choose other icons...still nowhere to enter my own notes.
Do you get the regular note window, when you disable auto rate in settings?

Are you changing the note icon settings for the right HUD?

Yes and the problem persisted on all huds.
However I did something that managed to fix the issue at least for now :-
I just disabled notecaddy , restarted HEM and then the regular notes were available.
I then enable notecaddy and everything after that worked fine
