Is it possible to transfer my notes from Fulltilt to HEM and visa versa?

No, not currently. This used to be possible to do on Pokerstars and we planned to add support for Full Tilt. Then Pokerstars changed their note system and broke that functionality, so we now need to add support for both sites.
