My HEM randomly misses a bunch of tournament cashes (but sometimes gets it correct) and puts it in the data view and results summary as not finishing ITM. Is there any reason for this or a way to fix it?

Which site and which HM version?
Are you using any additional poker software?

latest HM1, Stars and no additional software.

Stars writes separate tourney summaries which you need to import into HM:

For older tourneys you have to request the summaries (in english!) manually: Pokerstars > Requests > Tournament Histories...
