Did you purge any hands from database? Purging can break replay
Have you reset notes since 2.5 release? In some cases it is necessary

Create a new database. The old database is deleted.

Unfortunately there's no good logging for this error as nobody has ever reported it before. It will need to be added for the next beta build. The official thread will be updated http://forums.holdemmanager.com/note...-5-1-beta.html when there is a new build released which will probably be towards the end of the week. My apologies for the inconvenience
