what's the diff between
fold to flop continuation bet raised pot
and
fold to flop continuation bet
is it that the second one is in a limped pot?

fold to flop continuation = normal pot
fold to flop continuation Raised Pot = 3 bet pot where you were 3 bet preflop
fold to flop continuation 3 Bet Pot = 3 bet pot where you were the preflop 3 bettor--------------------------------------------------------------------------------------------------------------------

"% bet when checked to in a raised pot"

Not exactly, but you might find bet vs skipped continuation bet (CB) helpful.
